How It Works
After the initial verbal agreement, a Portable Smiles Mobile representative will conduct your site appointment to handle all necessary paperwork and scheduling. Information like the number of students enrolled and the school schedule is collected. Please note that students must be age 1 or older to receive services.
Schools are provided with official consent forms to send home to parents. Your representative will return to collect completed forms prior to the service date and the service date will require the use of a small room, preferably water source though we are flexible and can bring our own. Each child´s visit lasts an average of 15 minutes and each child will receive a toothbrush, toothpaste, and dental report form to take home. A proof of dental examination form will be left for each child at your site.

Cost & Frequency
There is NEVER A COST to the school or parents. Children enrolled in medicaid\Delta Dental, all kids & MI children are covered at 100%, and we can also bill private insurance. Parents must provide coverage information on the consent forms.
Preventative dental service is recommended every 6 months, so we coordinate our return visit every 6-months.
